We will provide you with transition information, resources and tools to start planning for the future. Learn how high school classes and the school environment can help develop important skills and build pre-employment knowledge.
Afternoon session with Lisa Cook Gordon from Michigan Family Voices
This workshop will give an overview of Medical Transition, including
* Planning and identifying future goals
* Objectives and concerns for the future
* Barriers to Medical Transition
Participants will be provided with medical transition health care skills checklists
