Community Partner Collaborative


What is the MI F2F Community Partner Collaborative?

The MI F2F Community Partner Collaborative is made up of individuals/agencies/organizations who provide services, support, and information to families of children and youth with special health care needs (CYSHCN) across Michigan (e.g., disability, social service, special education, early childhood, mental health, peer support, and disease specific organizations).

Members are invited to attend based on their interest in working together to promote a system of care for CYSHCN in Michigan. In addition to professional representation, we also invite family representatives to have a voice on the Collaborative as well. This could include agency/organization representatives that are parents or family members of CYSHCN as well as parents or family members with experience and interest in community engagement.

What does the MI F2F Community Partner Collaborative do?

  • Meet 4x per year.
  • Share activities, resources, and information with each other about services/supports for families of CYSHCN as it relates to emerging health trends.
  • Help build and share MI F2F online resource repository and resources/learning opportunities.
  • Identify opportunities to collaborate to break down silos of care that are a barrier to families to access services, particularly for underserved communities.
